Nolan has never to my knowledge ever done reshoots. Hell he doesn't even film with a 2nd unit. Nolan is very sufficient when it comes to filming. It's part of the reason why his films always come in under budget and finish ahead of schedule.
I think he manages to work with what he has. If you go over TDK's Pruitt building sequence with a fine-toothed comb, you see repeated hostage shots that would have been easy to do pick-ups with. But Nolan said "Eff it" and put it in anyway, because he figured it wouldn't ruin the momentum so why bother. It is a little distracting when you watch TDK more than once, but it really isn't that bad. It actually makes me admire Nolan more for not doing the pick-ups even though it was probably within his budget, but he realized that it probably wouldn't be worth the effort.

Also look at the scene where Alfred is patching Bruce up towards the beginning of the film. Pretty noticeable continuity error there.
That ALWAYS bothers me.

Me too! I always point it out when I'm watching it with someone and they don't know what I'm talking about.

Why on earth would you point something like that out to someone that doesn't notice it? You're ruining a perfectly good scene for that person.

Its like that scene in Raiders of the Lost Ark where Indiana Jones looks up in the snake pit and there's a cobra looking right at him... and if you squint you can see his reflection in the glass separating them.... its fun to find tricks like that.
I wouldn't say something like that "ruins" a film... its like finding easter eggs.
