I think it's one of Bane's mercenaries/LOS members that he elbowed (judging by the jacket and cap). As for where/when, it was dark and there was a light hanging behind them. Possibly part of the sewer fight when he teamed up with Catwoman? I wouldn't be surprised because the scene was only like a minute long (barely) and there was other stuff cut, such as that one really good shot of Catwoman going down the stairs.

At 0:18 we see Bane climbing , which isn't in the movie.
There are a few shot's from the trailers that weren't used in the final cut of the film. Also, we know from the ninja footage that there are some shot's that were filmed that weren't used in the trailers or the film.
